What is this resource?

The Preparing for Adulthood resource from PACER Center offers information and guidance to help youth with disabilities (ages 14–21) and their families prepare for the transition from school to adulthood. It focuses on transition planning through the Individualized Education Program (IEP) process and covers topics such as working with the IEP team, developing self-advocacy skills, understanding available transition services, preparing for changes when a student turns 18, and supporting engagement in the home and community.

How to use

This resource can be used by youth with disabilities, families/caregivers, and education professionals to guide transition planning and identify the skills, supports, and services needed for adulthood. It can help families prepare for IEP meetings, encourage youth involvement in decision-making, and explore strategies to develop autonomy and self-advocacy skills related to future goals.
