Transitioning from Pediatric to Adult Care While Transitioning to Undergraduate from Generation Patient is a resource guide designed to support students with chronic medical disabilities and rare diseases as they transition from pediatric health care to adult health care while beginning college. The resource provides information on finding and establishing care with a new adult health care provider, preparing for the first appointment, transferring medical records, communicating with a new care team, and understanding health care needs while living more independently.
