Educational Videos on Pediatric to Adult Healthcare Transition

The Center for Transition to Adult Health Care for Youth with Disabilities offers a collection of resources designed to help youth and young adults with I/DD prepare for the transition from pediatric to adult health care. The resources were developed to build health management skills and strengthen self-advocacy. The site offers tools, videos, quizzes, workbooks, guides, and timelines that provide information and support for young adults and caregivers to build agency in communicating with health care providers, understanding health needs, preparing for appointments, and making health care decisions.

This site provides resources for individuals, families, and professionals preparing for or supporting the transition to adult healthcare by assisting users in setting transition goals, developing important healthcare skills, and accessing tools to better understand and navigate the transition process. It can be used throughout the health care transition process and revisited as needs change.
