Services/Programs Description

Supported living solutions so men and women can live more independently.

Provides the following through the Innovations Waiver Services:
- supported living
- residential supports
- community living and supports
- community networking
- supported living transitions
- community navigator

Also can provide state funded services, B3 services, or private pay options.

Eligibility

Individuals with intellectual disabilities and/or chronic medical conditions in/around Wilmington and Greenville.
