How to use
This tool is a resource to help people with disabilities, families, and personal support networks work together to reach the goal of a good life for focus persons and peace of mind of parents and caregivers. This tool is not designed to be used by families in isolation without the support of a Facilitator. As a trained Facilitator, it is your role to guide the meetings by using this tool and by providing follow-up support. Be sure to familiarize yourself with the Meeting Planning section so that you have a sense of the general flow of these meetings. Facilitators must have skills and training in the development of personal support networks. PLAN Institute’s “Personal Support Network Facilitation” course is recommended
