Care Management is a central element of Managed Long-Term Supports and Services. In North Carolina, the new “Care Extender” service definition offers the opportunity to employ people with I/DD and family members as valued team members who can offer a unique and critical perspective on whole-person care. Identifying strategies that can enhance success and minimize potential challenges can increase the likelihood of success for the member, care extender, team, and Tailored Plan. This session will share perspectives and recommendations compiled from over 50 interviews with individuals with I/DD who will receive services, family members, LME-MCOs, I/DD, and healthcare providers, and policymakers. Lessons learned from Behavioral Health services and other states will be shared as we choose to accept this mission and commit to success.
