Services/Programs Description

Community Networking Transportation - Code H2015 U2It is a service billed on a per trip charge. Interested families should contact their assigned Alliance Care Manager to request this service. Just like all other services, it will need to go through the UM (Utilization Management) approval process and approval/funding amount is determined case by case.

Non-Emergency Medical Transportation (Modivcare) can used to and from Physical and Behavioral Heath appointments.

Eligibility

Community Networking Transportation: This is a state level policy and service offered for Innovation Waiver members only.

Modvicare: This is a benefit/service offered through Alliance to all members.
