The Help Hub offers:
– Assistance with the process of applying for Medicaid, SSI, or connecting with DSS and disability resources.
– Links to doctors, dentists, behavioral health and vision providers, and other specialists.
– Medication education and help with financial assistance.
– Connections to resources for food, housing, jobs, and much more.
– Referrals for care and durable medical equipment (DME).
– Education on lifestyle changes.
– Education to improve health and conditions.
