At Able Transition, we provide a range of services, from transition support, person-centered planning, long-term care planning, to, school transition services, skill building support, transition planning, and more.

ACHIEVE is a statewide, virtual Pre-ETS program designed for students with disabilities, ages 14 to 21. This program is available to those enrolled in an educational program who are eligible or potentially eligible for EIPD services and have a recognized need for Pre-Employment Transition Services. ACHIEVE is accessible to students in various educational settings, including those who are homeschooled, or enrolled in traditional public schools, charter schools, virtual schools, private schools, early college programs, vocational schools, college courses, or training programs. The ACHIEVE Transition Program offers a comprehensive curriculum designed to equip students with the skills and knowledge they need for successful transitions from school to work and into adult life.

This organization provides support for adults with intellectual and/or developmental disabilities as well as their sibling care partners. Services include natural supports education, community networking, assistance for navigating Medicaid funded services, and peer support/coaching.

The LME-MCO Alliance Health can help you access Flex Funds. Flex Funds are intended to supplement informal/volunteer resources and to improve a person’s quality of life and inclusion in the community. These funds should be requested to assist with an individual’s social drivers of health as it relates to engagement and self-management.

ADA strives to enable people to with any disabilities to fully participate in their communities without boundaries in the Raleigh-Durham and surrounding area. They offer advocacy, disaster response, independent living services, information and referrals, peer support, reentry, and transition support.

Autism After 18 creates opportunities for young adults with autism to interact in the community on a social, professional, and educational level.

Autism Consulting of the Carolinas supports families as they navigate the complex emotions and experiences of caring for a child with ASD by empowering clients with the tools needed to work through these difficult emotions and discover ways to maximize the wonderful potential of their children. They also offer services related to transition planning, including considerations of post-secondary education and entering the workforce.

Autism Grown Up, Inc. is a non-profit organization that is entirely online, which means that all of our resources are available online so we are able to reach the autism community globally. Resources include: a free lifespan toolkit, visual recipes, transition planning guidebooks We create and provide resources for autistic people and all members of an autistic person’s support network, including family members, teachers, clinicians, therapists, professionals, peers, employers, and other important stakeholders. This is the crew that surrounds an autistic person as they grow up and navigate adulthood. We all don’t do it alone!
